The whole kit: one kit, three forms
Phone app (this app): the daily record, in your pocket. Type here first, the same day, at the office, the pharmacy, the store.
The Workbook (computer file): the same records on a laptop. For big tables, and the monthly backup.
The Print Pack (paper): the folder you carry to the PDAO office and the pharmacy counter.
Phone is the daily record. Workbook is the monthly backup. Five minutes, once a month: open Backup (bottom of the screen), Export your backup file, and save the tables into the workbook tabs with the same numbers.
The numbers match, in all three forms

The rules of this kit
This kit is for records and preparation only.
It never decides if you qualify for a PWD ID or a discount. That decision belongs to your PDAO and your doctor.
Your PDAO and the current NCDA order win over anything written here. If this app and your PDAO say different things, follow your PDAO.
The trackers in this kit do not replace your official purchase booklet. Always bring the real booklet.

Update promise: corrections and rule-change updates are free for changes that take effect on or before December 31, 2027.
The workbook and printed guide stay as the laptop and print companion to this app. They hold the same records in file form.

07 Weekly 5 Percent Tracker
The weekly ceiling
The 5 percent discount on basic necessities and prime commodities (DTI-DA-DOE Joint Order 24-02) has a hard weekly ceiling:
Only the first 2,500 pesos of each week counts, Monday to Sunday. 5 percent of 2,500 is 125 pesos, the biggest discount in any week.
What you do not use does not carry to next week. Every Monday starts at zero. This discount has no VAT exemption, and it is separate from the 20 percent discount.
Mon, Nov 2, 2026: 800 pesos. Puregold Santa Maria: rice, cooking oil, canned goods.
Week total 800. Discount so far 40.00.
Wed, Nov 4, 2026: 950 pesos. Puregold Santa Maria: milk, eggs, bread, detergent.
Week total 1,750. Discount so far 87.50.
Fri, Nov 6, 2026: 900 pesos. Palengke, Santa Maria: vegetables, chicken, soap.
Week total 2,650. Discount so far 125.00.
The week reached 2,650, but only the first 2,500 counts, so the discount stops at 125.00.
Mon, Nov 9, 2026: 600 pesos. Puregold Santa Maria: rice and canned goods, NEW WEEK.
Week total 600. Discount so far 30.00. The new week starts at zero. Nothing carried over.

08 Discounts: Receipt and Incident Logs
Receipt log
For the 20 percent discount (RA 10754), your receipt should show all 4 marks:
- Your name
- Your ID number
- The VAT-exempt mark
- The less-20-percent mark
Log each receipt and tick whether all 4 marks are there. The log flags any receipt that is missing one.
Oct 18, 2026, Jollibee Santa Maria: lunch for Maria. Price 560, VAT removed 60, discount 100, paid 400.00. All 4 marks: Yes. Complete
Oct 25, 2026, St. Clare Laboratory: blood test. Price 900, VAT removed 96.43, discount 160.71, paid 642.86. All 4 marks: No. Check the receipt: something is missing
How the second one works: a 900 peso lab test. VAT removed 96.43, then 20 percent off the rest (803.57) is 160.71, so Maria paid 642.86. The receipt missed one of the 4 marks, so the check flags it.
Your receipts
Add a receipt
If your discount is refused
Rules as of November 2026. This is information and record-keeping only. It is not legal advice, and this app cannot argue your case for you.
Some refusals are correct
A store or pharmacy may lawfully refuse or cut the discount when: you have no ID with you, or the ID is expired; the name on the papers does not match the ID; the purchase is not for your own exclusive use; there is no prescription for a medicine purchase; the quantity is beyond a 30-day requirement; or, for PWD medicine, you did not bring the purchase booklet. If the refusal was for one of these reasons, fix the paper problem and go back. Write it in the log anyway.
If the refusal looks wrong: keep the evidence
- Keep the receipt, even a receipt with no discount on it.
- Take a photo of your ID beside the receipt.
- Write down the date, the store, the staff name if you got it, and what was said, in the log below, the same day.
- Do not sign anything you do not understand.
Where complaints go
| Problem with | File with |
|---|---|
| A store, grocery, restaurant, or other retailer | DTI (Department of Trade and Industry), consumer complaints |
| A pharmacy, hospital, laboratory, or clinic | DOH (Department of Health) |
| Your card or your record itself | Your issuing LGU: the PDAO or CSWDO/MSWDO that issued your card |
Sep 14, 2026, pharmacy near the public market: refused the medicine discount because Maria left the purchase booklet at home. The booklet IS required for PWD medicine purchases, so this refusal was correct. Lesson recorded, not a complaint. Evidence kept: none needed. Filed: none. Outcome: booklet now lives in the wallet pouch.
Incident log
Add an incident
On paper: Workbook tab 08 and tab 10, Print part 6 (if refused) and part 8 (incident form). Kit 08 and 10.
09 My Rep: Representative Register
The golden set
Before the letter: your representative must carry all four. A store may not lawfully refuse a properly documented representative, and an undocumented one will be refused.
| 1. | Your ORIGINAL PWD ID (not a photocopy) |
| 2. | Your purchase booklet |
| 3. | Your representative's own government ID |
| 4. | Your authorisation letter, signed by you, dated within the last 7 days |
Under DTI-DA-DOE Joint Order 24-02, an authorisation letter is normally good for 7 days from the date you sign it. Sign a fresh one each week your representative will buy for you. The letter itself is in the Guide.
Ana S. Reyes (daughter). Her ID: driver licence, N01-12-345678 (example). She may: buy medicine and groceries for Maria. Letter signed Nov 2, 2026, so it expires Nov 9, 2026. Lives next door. New letter every Monday.

The move to the Unified PWD ID, step by step
Source: NCDA Administrative Order No. 001, series of 2026, and NCDA's November 4, 2026 briefing.
Step 1: Everyone must register
Every PWD must register in the new unified system (UPWDIS), even if your old card is still valid. About 2.7 million holders still have to register. The new card has a QR code, so a store can check it. That is the government's answer to fake IDs. Finishing your move is your protection when a store checks your card.
Step 2: Find your office
Register at the Persons with Disability Affairs Office (PDAO) of your city or town. Only about 400 to 500 of the 1,715 local governments have one. If yours has none, go to the City or Municipal Social Welfare and Development Office (CSWDO or MSWDO). It does the same job. Write your office down once, on the My Card screen. Far areas also get assisted and mobile registration teams; ask your barangay if one is coming near you.
Step 3: Pick your lane
Lane A, disability that can be seen: no doctor's certificate is needed. The officer assesses you at the office.
Lane B, disability that cannot be seen: get a Certificate of Disability first, from a specialist or the right doctor (city, municipal or regional health office, or a recognised private doctor). For cancer and rare diseases, a doctor with the right expertise is enough. Go to the PDAO only after the certificate. The Checklist screen holds both lanes.
Step 4: Bring everything, once
Tick every paper in your lane on the Checklist screen, then bring the whole folder. Repeat trips happen when one paper stays home.
Step 5: Know what happens to your old card
There is no official deadline yet for old cards. The move runs three years; the government targets full registration by 2027 and expects old cards phased out by 2030. Targets, not deadlines. Your old card is not automatically valid for life, and not automatically void. Keep using it until your PDAO says otherwise, and finish your registration so the question stops mattering.
Step 6: Know your validity
Permanent disability: the unified card is valid for life. Non-apparent disability: revalidation every 5 years. Some holders are both at once. If that is you, ask your PDAO which rule your record carries, and write the answer on the Dates screen. It works out your reminder dates.
Ask your PDAO (only your local office can answer these)
- Am I already listed in the unified registry, or do I register from the start?
- Which form do I fill in, yours or the DOH form?
- Do you ask for a whole-body photo?
- Is my record lifetime, or 5-year revalidation?
- When my card is ready, how will I know, and what do I bring to claim it?
- My disability cannot be seen. Which doctors near here can issue my Certificate of Disability?

For PWD medicine purchases, the purchase booklet is STILL required.
In 2024 the booklet rule was removed for senior citizens only. It was not removed for persons with disability. Many holders heard "the booklet is gone" and stopped bringing it, then were refused at the pharmacy. The refusal was correct. The two booklets now follow different rules:
| PWD booklet | Senior booklet | |
|---|---|---|
| Still required? | YES, for medicine purchases (DOH AO 2017-0008) | No, removed in 2024 |
| What to bring | PWD ID, booklet, prescription | Senior ID, prescription |
If your booklet is lost or filled up, ask your issuing office for a replacement. The Medicine screen in this app keeps a copy of your booklet history, so a replacement request is backed by your records. Remember: the app copy supports the booklet. It never replaces it.

Two different discounts. Do not mix them up.
Sources: RA 10754 and its IRR; DTI-DA-DOE Joint Administrative Order No. 24-02, series of 2024. Rules change; check the date on this page.
Discount 1: 20 percent, plus VAT exemption (RA 10754)
On these, for your own exclusive use:
- Medicines (bring ID, booklet, prescription)
- Medical and dental services, diagnostic and laboratory fees, and professional fees of doctors
- Hotels, lodging houses, and similar places
- Restaurants (food and drink for your own consumption)
- Recreation centers: movies, concerts, sports events
- Land, air, and sea travel inside the Philippines
- Funeral and burial services (arranged by the family)
The VAT exemption goes with this discount. Your receipt should show your name, your ID number, the VAT-exempt mark, and the less-20-percent mark. The Discounts screen checks all four.
Discount 2: 5 percent on basic necessities (separate rule)
On basic necessities and prime commodities only: rice, corn, bread, fresh or canned fish and marine products, fresh pork, beef and poultry, fresh eggs, fresh milk, fresh vegetables and fruits, root crops, coffee, sugar, cooking oil, salt, laundry soap and detergents, firewood and charcoal, and household LPG and kerosene.
The weekly ceiling: add up your basic-necessities purchases for the week, Monday to Sunday. Only the first 2,500 pesos counts. 5 percent of 2,500 is 125 pesos, the biggest discount in any week. What you do not use does not carry to next week. Every Monday starts at zero. This discount has no VAT exemption, and it is separate from the 20 percent discount. The Weekly 5% screen does this arithmetic for you, purchase by purchase.
On paper: Print part 5. Kit 13.
Also yours: PhilHealth is already yours
Source: RA 11228.
Under RA 11228, every Filipino person with disability is automatically covered by PhilHealth. You do not pay the premium; it is paid from the national government's share of sin tax revenues.
- Coverage follows your registration in the PWD registry. Finishing your move to the unified system keeps your record current.
- Many holders do not know this coverage exists. Now you do.
If a hospital or clinic says you are not covered
- Show your PWD ID and ask them to check your record again under RA 11228.
- Ask for the hospital's billing or PhilHealth officer, not only the cashier.
- Write down who you spoke to, and the date, on the Discounts screen (Incident Log), the same day.
- Confirm your status with PhilHealth directly before you pay any premium anyone asks you for.
This page is a note, not a PhilHealth document. PhilHealth's own record wins. Confirm your own status with PhilHealth before you rely on this page.
On paper: Print part 5, last page. Kit 14.
If your discount is refused
This page is information and record-keeping only. It is not legal advice, and this app cannot argue your case for you.
First: some refusals are correct

The authorisation letter (fill in, sign, date)
Under DTI-DA-DOE Joint Order 24-02, an authorisation letter is normally good for 7 days from the date you sign it. Sign a fresh one each week your representative will buy for you. The golden set is on the My Rep screen.
Date: ____________________
To whom it may concern:
I, ____________________, holder of PWD ID number ____________, authorise my representative,
____________________, holder of government ID ____________ number ____________,
to buy medicines and basic necessities for my own exclusive use, using my PWD discount and my purchase booklet, on my behalf.
This letter is valid for seven (7) days from the date written above.
Signed: ____________________ (the PWD ID holder, signature or thumbmark)
Printed name: ____________________
Record every letter on the My Rep screen. It works out the expiry day and warns you before it lapses.
On paper: Workbook tab 09, Print part 7. Kit 16.
Your revalidation reminder
Only for holders whose card carries 5-year revalidation. If your card is lifetime, you do not need this page.
MY NEXT REVALIDATION
The Dates screen works out your exact dates: the revalidation date itself, plus reminders 6 months before and 1 month before. When a reminder month arrives, put the visit in your calendar and tell a family member, so two people remember. Go to your PDAO in that month. Bring your card, your folder, and your booklet.
If you are not sure whether your record is lifetime or 5-year, ask your PDAO and write the answer on the Dates screen. Your PDAO's record wins.
In the printed guide this page is a cut-out strip for the refrigerator door. On the phone, this page and the Dates screen do the same job.

Backup: Export and Import
One backup file, everything in it
Export makes ONE file holding everything you typed in this app: your record, your visits, every log, every representative. Keep that file somewhere safe, for example in your Drive folder. It is also what you carry to a new phone.
Bring a backup back (Import)
Warning: Import replaces everything on this phone now. What you typed here will be overwritten by the file. Export first if you are not sure.
Save a table for the workbook
Each button makes a table file whose columns match the workbook tab with the same number. Open the file, copy the rows, and paste them into that tab. Phone is the daily record; the workbook is the monthly backup.
The whole kit: one kit, three forms
Phone app (this app): the daily record, in your pocket.
The Workbook (computer file): the same records on a laptop. For big tables, and the monthly backup.
The Print Pack (paper): the folder you carry to the PDAO office and the pharmacy counter.
Phone is the daily record. Workbook is the monthly backup. Five minutes, once a month. The full numbered map, Kit 01 to 16, is on the Home screen.
